Primary Rhinoplasty

The nose has a very important role in breathing. Besides, a nice looking nose provides balance to the face and a more pleasant appearance.

Rhinoplasty is the most commonly performed cosmetic surgery, however it is the most difficult one. Some surgeons may achieve a good result with minor changes, but it takes an experienced surgeon to achieve a reproducible and predictable outcome for every nose and to direct you toward making the right decision.

There are mainly two approaches, open and closed rhinoplasty. The nose is a very complex structure including skin, muscle, soft tissues, ligaments, cartilage and bone. One should bear in mind when performing a rhinoplasty that this is a very delicate surgery whereby cosmesis and function interplay and it is very important to seek a specialist in the field to guarantee the outcome. You should be over 16 if you are planning to undergo a rhinoplasty. At this age, the nasal bones are mature.

The surgery takes approximately 4 hours and it is done under general anesthesia.

Am I a good candidate?

The following are the most common reasons to undergo rhinoplasty:

  • Nose size out of balance with the rest of the face
  • Nasal tip that poorly defined
  • Nasal asymmetry or crooked nose
  • Wide nose.
  • a visible bump (dorsal hump)
  • Difficulty breathing secondary to structural issues such as a deviated septum.

What is the recovery period?

 You will have a cast for 7 days and you should expect minimal swelling and bruising that gradually improve within 14 days. If the surgery was done using the open technique, stitches will be removed after 1 week. You will be able to go back to work in 10 to 15 days.

At 3 months, you feel good about yourself, at 6 months you look good in the pictures and at 1 year you are only 80% healed. It takes 3 years to be fully healed.

What about the scars?

Dr. Abi Akl performs the open and closed approach. Small improvements can often be accomplished with the closed (endonasal) approach resulting in no external scar while complex noses require the open approach. While the open approach does result in a small scar in the columella, it will be invisible after a few months with proper closure technique.

At the time of the consultation, Dr. Abi Akl will identify where the problem is, taking into consideration the airways and the cosmetic appearance. There is nothing that cannot be fixed but the surgery should be customized to each individual. Dr. Abi Akl can lengthen or shorten the nose, soften the tip, augment or reduce the dorsum.

Alarplasty

The nostrils may need to be refined in some cases where they look uneven or bulky or in patients with a large flare. Different techniques have been used but the results were not really satisfying specially with the scarring and the operated look.

Dr. Abi Akl’s technique resides in doing incisions along the nasal base crease keeping the alar facial groove intact and without disturbing the sill or the nasal base. The incision is hidden in the natural crease and after you have completely healed, the scar will be almost invisible. The procedure can be done at the same time of the rhinoplasty or separately. In this case, local anesthesia is enough.

Revision Rhinoplasty

The nose provides balance to the face and it is of utmost importance to preserve a natural look after rhinoplasty. Unfortunately, sometimes you may end up with a nose that you did not want or simply that doesn’t fit your face. Dr. Abi Akl is an expert in revision rhinoplasty.

If you are not satisfied with the appearance of your nose after a previous rhinoplasty, revision rhinoplasty may be a good option to restore your facial harmony and confidence.

In revision rhinoplasty, cartilage grafts are often necessary to rebuild the nose. Most of the time, there is minimal amount of cartilage left from the previous rhinoplasty. However, before trying to get cartilage from the ear or the rib, Dr. Abi Akl will try to maximize the amount of cartilage from the nasal septum. If absolutely necessary, Dr. Abi Akl will use cartilage grafts or fascia from the ear or even the rib to obtain the most optimal result.
